Please read our policies:

Contacts

• These fees cover the initial contact lens fitting process, trials in the cases of soft contacts, starter solution and follow up visits for up to 2 months.
• Fitting fees are due AT FULL at your initial fitting appointment.
• CUSTOM CONTACT LENSES: All custom contact lens orders require a deposit of 50% before we can order the lenses. When we finalize the prescription, the remaining balance is due.
• CONTACT LENS PRESCRIPTION: Your prescription is valid for one year from date of your initial contact lens examination. Contact lens prescriptions are not the same as glasses prescriptions. This Federal Guideline is managed by The Fairness to Contact Lens Act ((Pub. L. 108-164,117 Stat.2025, 2026, 2027, 2028 and 2029, codified at 15 U.S.C. ch. 102 et seq.), also known as FCLCA).  
• All fitting fees and open balances must be paid before we release your prescription.
• Custom contact lenses can take a few days or weeks to be manufactured. We will be glad to update you on the progress of the lenses if you would like.
• Please ask an associate for details on your contacts and any rebate offers.
• CONTACT LENS FOLLOW-UPS: We take our follow-up appointments very seriously; therefore, it is your responsibility to keep your appointments in order to finalize your contact lens prescription. If you take longer than 3 months from initial fitting to finalize your contact prescription, you will be charged $45.00 for a re-fit examination.  
• You cannot order contacts until you let us know and confirm that they are adequate. This is considered as finalization of contact lenses.

Insurance

• Dear Patient: Thank you for choosing us! Our mission is to deliver the highest quality of eye care you will ever receive. As a courtesy, we will be happy to bill your insurance company for your visit with us. But please understand that this is your insurance policy.
• Please read our insurance policies, sign and date. By signing, you agree to pay your claim if for any reason your insurance company denies it.
• INSURANCE CARDS: Please have your card with you so we can copy all the necessary information. This will make the billing process easier for all of us. If your insurance should change in the future, please inform the office prior to your next visit.
• CO-PAYS AND DEDUCTIBLES: Have your payment ready when checking out at the front desk. This is a contractual obligation with your insurance company for which you are responsible and its mandatory that we collect it from you.
• REFERRALS: As a member of an HMO, you have become a partner with your primary care physician and it is mandatory that you get a referral from your primary care physician to see a specialist if you are required to. Optometrists are considered specialists. We may require referrals for all visits that are not routine in nature, i.e. itchy red eyes, an injury to your eye(s), seeing light flashes or floaters, and so on if your insurance requires it. If possible, obtain multiple visits in your referral so you do not have to repeat this process if we need to see you more than once. If a referral has not been obtained and the claim is denied, you will be responsible for payment in full.
• INSURANCE BENEFITS: There are many insurance companies and many plans within insurance companies. Therefore, it is in your best interest to verify your benefits. Know your deductible amounts. Please also verify that you do not need a prior authorization for your visit. Many Vision programs do not issue their own insurance cards, so it is crucial that you are aware of your Vision Program prior to your visit as your health insurance card will not inform us of separate vision programs.
• WORKER’S COMP: We do not bill Worker’s Comp. cases. Payment is expected in full at the time of the visit. We will be happy to assist you in any paperwork or forms that need to be filled.
• SELF-PAY: Payment is made in full at the time of the visit.
• CANCELLATION/ NO SHOW POLICY FOR DOCTOR APPOINTMENT: We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. If an appointment is not cancelled at least 24 hours in advance, you will be charged a forty dollar ($40) fee; this will not be covered by your insurance company.
• CONTACT LENS FITTINGS: If you elect to try contact lenses, a separate fitting exam must be performed. This must be paid for at the time of service. The fitting fee includes the initial fitting visit; training time, sample solution and trial soft contact lenses, and all the follow up visits for the first 3 months. This fee is non-refundable. These fees don’t include your supply of contacts. We can exchange unopened, undamaged boxes of contacts that are not expired ONLY.
• FRAMES, LENSES and YOUR PRESCRIPTION: All of our frames have a manufacturer’s defect warranty ranging from 6 to 12 months. Please ask for more details on other warranties and policies.

Our staff will ABSOLUTELY accommodate you through any of the processes listed above to their best efforts. We understand that insurance policies and programs can be quite confusing. Insurance tends to change every year and many do so without informing you, the patient. Your patience and understanding will be greatly appreciated in helping to resolve any problems.

Medical Testing and Imaging

Dr. Kahen-Kashi continues to acquire and use today’s most advanced technologies to provide patients with the most thorough eye health evaluations possible. Medical testing and imaging will allow the detection of certain eye health problems at the earliest stages, often before a patient would be aware something was wrong with their vision. Detecting potentially blinding diseases in the earliest possible stages always provides the best chance for preventing permanent vision loss.  With such testing, you will be more accurately and efficiently evaluated for:
